Please use this form to send a surgery reservation for your pet. Requests must be made at least 7 days in advance and will be confirmed by telephone or email within two business days. Surgery patients must have no food after 10:00 PM the night before surgery. Water is to be withheld early of the morning of surgery. Surgeries are performed Monday through Friday.
